What you'll do

• Conduct nutritional assessments for new admissions and quarterly, with more frequent reviews for high-risk residents
• develop, implement, and monitor individualized nutrition care plans
• provide nutrition education and counseling
• collaborate with physicians, nurses, therapists, and other care team members
• maintain documentation and regulatory compliance
• monitor resident weight
• participate in quality improvement as needed
• conduct monthly kitchen sanitation audits
• review and approve menus semi-annually.
