START Care Coordinator

The role includes rotational on-call crisis support as part of the 24-hour START program.

What you'll do

• Complete START screenings, intake assessments, service evaluations, and required documentation
• maintain a caseload and coordinate support
• conduct outreach in homes and community settings
• coordinate meetings and collaborate on crisis plans
• monitor progress and risk factors and adjust coordination strategies
• provide rotational on-call crisis support
• educate and support individuals and families
• maintain secure records and complete required START training.
