What you'll do

• Provide preventative and rehabilitative nursing care
• monitor and report changes in client condition
• educate and counsel clients and caregivers
• document services in the EMR
• understand payer and authorization requirements
• complete supervisory visits with LPNs and HHAs
• direct skilled nursing services in accordance with standards and regulations
• communicate with clients, families, physicians, referral sources, and the care team
• participate in team meetings and projects.
