Job overview
The Registered Nurse (RN) Case Manager role is onsite in Winchester, MA, with pay of $45–$55 per hour. Winchester Rehabilitation and Nursing Center’s case manager works with the Director of Nursing and interdisciplinary team to support residents and families through care.
What you'll do
- Monitor HMO patients’ level of care at admission and during their stay
- obtain prior authorizations for transportation, therapy, equipment, and other services
- track Medicare and HMO days
- collaborate on resident care plans
- participate in interdisciplinary and family meetings
- and support compliance with Medicare, Medicaid, PPS, PDPM, MDS, and regulatory requirements.
