Youth & Young Adult Coordinator

Job overview

The Youth & Young Adult Coordinator role is onsite in Akron, OH, and requires residence in the Medina or Summit County catchment area. Compensation is not specified. Bellefaire JCB’s OHTP program works to improve access to treatment and support for youth and young adults ages 16–25 with serious emotional disturbance or serious mental illness. The coordinator supports outreach, engagement, referrals, care coordination, and treatment services for young people and their families. The role connects participants with navigators and collaborates with program staff while maintaining clinical documentation and client confidentiality.

What you'll do

• Participate in or lead outreach to transitional-aged youth and families
• connect them with TAY Navigators
• accept supervisor-assigned cases to provide Medicaid-reimbursable Community Psychiatric Supportive Treatment and/or Therapeutic Behavioral Services as needed
• engage clients, families, and supportive adults in treatment planning
• complete clinical documentation
• protect client privacy and follow agency policies.
