What you'll do

• Develop and maintain relationships with hospitals, physicians, discharge planners, case managers, social workers, and community organizations
• generate referral sources and strengthen partnerships
• visit referral partners in the territory
• represent facilities at community events
• explain facility services
• collaborate with Admissions, Clinical Leadership, and Executive Directors on transitions
• monitor referrals, admissions, and market trends
• maintain marketing and referral records
• travel between facilities and surrounding communities.
