What you'll do

• Provide in-home care to patients with illnesses, injuries, and chronic diseases
• work with physicians and oversee LPNs, CNAs, and aides providing care
• assess patients, document progress, and report changes to physicians
• educate patients on care plans, medication, therapy, diet, and exercise
• coach families on care plans, medication, and home safety
• ensure goals are met and coordinate discharge from services. Typical caseload is 30–35 patients, with an average of 5–7 visits per day.
