What you'll do

  • Coordinate each patient’s stay from admission through discharge
  • work with the interdisciplinary team to identify goals and address discharge barriers
  • explain rehabilitation processes, levels of care, and patient rights and responsibilities
  • advocate for patients within CMS and managed-care guidelines
  • connect patients with next-level care resources
  • maintain patient and family contact about progress and discharge planning
  • anticipate and address changes in disposition.
