Clinical Health Educator (RN) - PRN

Facilitate/teach targeted diagnosis-specific education and Disease Self-Management Courses. Perform health screenings, including phlebotomy and flu vaccinations. Provide in-patient hospital rounding for patient education, out-patient support coordination, and family/caregiver engagement. Participate in Quality Improvement Processes.
