Registered Nurse (RN), Hospice Visits (PRN)

The Registered Nurse (RN), Hospice Visits (PRN) role is onsite in Dixon, IL, with coverage across Lee, DeKalb, Ogle, and surrounding counties. Pay is $40–$45 per hour. The Extended Care Team supports Transitions Hospice patients and families when additional care is needed. The nurse provides flexible clinical reinforcement across cases, assessing and treating patients, coordinating with the interdisciplinary team, and helping ensure patient needs are addressed without delay.

What you'll do

• Provide nursing care across the nursing process within scope of practice
• assess patients and families and contribute to individualized care plans
• complete visit types as needed
• administer ordered medications and treatments
• identify nursing diagnoses
• document clinical notes and pain and symptom management outcomes
• communicate condition changes to physicians and obtain orders
• coordinate with community health partners
• arrange equipment and services
• participate in continuing education and in-service training.
