The Registered Nurse Case Manager — Full-Time Days role is onsite in Victoria, TX, serving the Victoria and Port Lavaca area. Compensation is not specified. Hospice of South Texas is a nonprofit provider of compassionate end-of-life care. The schedule is Monday–Friday, 8:00 a.m.–5:00 p.m., with occasional on-call and weekend responsibilities. The nurse assesses hospice patients and caregivers, implements and revises individualized plans of care, and coordinates services with families, physicians, and the interdisciplinary team to support patient-centered end-of-life care.

What you'll do

• Conduct initial and ongoing assessments of patients’ physical, functional, psychosocial, and environmental needs
• implement and recommend revisions to individualized plans of care
• educate patients and families
• document clinical progress
• coordinate services with the interdisciplinary team
• communicate changes in patient needs
• determine service scope and frequency
• assess caregiver capacity
• apply hospice admission and recertification criteria
• provide assigned on-call coverage.
