What you'll do

• Measure and document member progress according to Individual Service Plans
• maintain confidential member information
• teach life skills and implement Person Centered Service Plans
• engage members individually and in groups
• support mobility, hygiene, supervision, and safety
• administer medications as permitted and trained
• complete required documentation
• report incidents and attend required training.
