What you'll do

• Assist with resident assessments and implementation of care needs
• participate in resident and family care conferences
• communicate with residents, families, staff, physicians, outside providers, and the public
• evaluate, monitor, and document changes in resident condition and implement interventions
• support medication management and safe medication dispensing
• monitor weight, nutrition, skin integrity, mobility, falls, and pain.
