Registered Nurse Case Manager

Compensation is $2,823 per hour. The Registered Nurse Case Manager role is onsite across Petaluma and Healdsburg, CA, with workdays divided between the two locations and every other weekend included. The role supports acute hospital case management and discharge planning. The nurse coordinates care and discharge planning, applies admission and utilization review criteria, and supports appropriate patient transitions in acute hospital settings. Epic experience is required, along with two years of acute care case management or discharge planning experience.

What you'll do

• Provide case management and discharge planning in acute hospital settings
• support care coordination and admission criteria review
• perform utilization review using InterQual criteria
• use Epic
• float between Petaluma and Healdsburg according to the varying weekly schedule
• work every other weekend.
