What you'll do

• Manage an assigned caseload from admission through discharge using assessment, planning, coordination, and evaluation
• complete comprehensive assessments, including OASIS when required
• develop and update individualized care plans
• provide skilled nursing for preventative, rehabilitative, palliative, and end-of-life needs
• educate patients and families
• coordinate with interdisciplinary team members
• supervise LPNs and paraprofessionals
• communicate condition changes
• secure equipment and supplies
• maintain timely electronic medical record documentation.
