What you'll do

• Monitor the safety and well-being of assigned clients
• provide referrals and direct services
• develop treatment and safety plans with families

• conduct home assessments and document findings

• maintain case records and reports
• transport youth and families as needed
• participate in a system of 24-hour availability for families in crisis.
