What you'll do

• Travel regularly throughout the company to provide patient care at agencies needing permanent staff
• perform initial nursing assessments for new admissions
• complete, coordinate, and revise plans of care
• communicate patient progress to clinical managers, physicians, and the care team
• maintain patient records
• record pain and symptom management changes and outcomes
• educate families
• make referrals to hospice disciplines as needed
• perform assigned on-call responsibilities.
