What you'll do

• Assess physical, mental, and emotional health and functional abilities
• collaborate with interdisciplinary teams on individualized care plans
• maintain relationships with hospitals, community agencies, social workers, case managers, and payor contacts
• educate residents and families about services and care expectations
• monitor progress and update plans
• coordinate with providers
• and document assessments and care plans.
