What you'll do

• Coordinate referrals, pre-admission screening, resident and family education, required documentation, and electronic medical record entry
• collaborate with interdisciplinary teams and providers on care planning
• coordinate discharge services, equipment, appointments, and external-provider communication
• maintain records and provide benefits, data-entry, and administrative support
• participate in quality assurance, performance improvement, and regulatory compliance activities.
