What you'll do

• Collaborate with the interdisciplinary team to assess, plan, implement, and evaluate resident care plans
• administer oral, subcutaneous, and IV medications and document responses
• provide bedside care
• monitor specialized equipment and clinical data
• respond to changes in resident condition
• perform wound care, catheter insertion, specimen collection, and other ordered procedures
• educate residents and families
• maintain clinical documentation
• participate in discharge planning and care coordination.
