Resident Services Assistant

What you'll do

• Assist with admission paperwork and gather information from residents and families
• help complete and document social services assessments and follow up during residents’ stays
• support discharge planning and coordinate paperwork, services, resources, and communication
• schedule appointments and coordinate transportation as needed
• work with nurses, therapists, and other team members
• participate in care conferences and follow up on questions or concerns
• maintain accurate documentation
• advocate for residents’ rights, preferences, and dignity
• keep the coordinator informed of appointments, discharge needs, incomplete paperwork, and concerns.
