Registered Nurse (RN) Care Coordinator

What you'll do

• Coordinate patient care as part of the PCMH Care Team
• identify patients for care coordination and prepare handoff tools
• review records and obtain missing documentation
• coordinate referrals, treatments, procedures, home health services, and admissions
• assess patient needs and develop care plans
• support disease self-management and medication adherence
• communicate with providers and care team members
• document interactions in electronic medical records
• provide patient education and follow-up calls
• monitor care quality and oversee assigned staff competencies.
