Physical Therapist (PT) - Travel Contract

Provide outpatient physical therapy, including evaluation, treatment planning, and documentation. Experience with functional treatment programs and delegation of discipline-specific therapy services. Complete comprehensive evaluations, goal writing, letters of medical necessity, and timely clinical documentation per payer and clinic guidelines.
