What you'll do

• Provide culturally responsive, person-centered, strengths-based, trauma-informed case management
• assess client strengths, needs, and goals
• develop service plans and track progress
• conduct risk screenings and assist with safety planning as appropriate
• connect clients with community resources and benefits
• provide in-office, community-based, and virtual appointments
• maintain case documentation
• protect confidentiality
• build relationships with community organizations and service providers.
