What you'll do

• Supervise and support nurses, home health aides, therapists, and medical social workers
• coordinate clinical operations and patient-care delivery
• conduct case conferences and assist with care planning
• review clinical documentation, OASIS assessments, and physician orders
• monitor outcomes, productivity, and quality indicators
• educate and mentor staff
• support quality assurance and survey readiness
• serve as a clinical resource
• assist with clinical policies and procedures.
