What you'll do

• Assess patients’ psychosocial status and communicate findings to the registered nurse and interdisciplinary group
• conduct social evaluations of family dynamics, caregiver abilities, communication patterns, and risks
• plan interventions and provide counseling or crisis intervention as needed
• assess and educate the team about cultural needs
• maintain clinical records and point-of-service documentation
• educate patients and families about advance directives
• provide information and referrals for practical and environmental needs
• liaise with community agencies
• collaborate with organization personnel
• participate in care planning, interdisciplinary meetings, and discharge planning when needed.
