What you'll do

• Collect and assess resident health and well-being information from admission to discharge
• plan, document, and submit information for care plans
• develop and maintain individual care plans
• maintain healthcare records and reports, including use of PCC
• investigate patient and family complaints and take appropriate action
• collaborate with staff, residents, and families.
