The Registered Nurse (RN) Afterhours Care Manager is an onsite role based in Henderson, NC, responsible for providing high-quality, compassionate hospice care outside standard business hours. This position ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. This role exists to provide essential clinical oversight and support for patients and families during critical after-hours periods. The Care Manager acts as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team.
