Care Coordinator

Job overview

The Care Coordinator role is fully remote, with a fixed Monday–Saturday schedule of 9:30 AM–6:00 PM in the supported time zone. Compensation is not specified. TimeDoc Health provides virtual care management to help patients with chronic conditions stay connected to care between office visits and support better health outcomes. The coordinator conducts phone-based chronic care management, educates patients, coordinates with providers and community resources, and documents care plans and interactions. The role supports continuity of care and helps patients remain healthy between appointments.

What you'll do

• Conduct monthly chronic care management calls
• make 30–50 outbound calls daily
• educate patients on chronic conditions and care plans
• coordinate with provider offices, pharmacies, and community resources
• identify care gaps and social determinants of health
• update personalized care plans in the EMR
• document interactions and maintain HIPAA compliance.
