What you'll do

• Complete Start of Care assessments and skilled nursing visits, including wound care, medication management, patient education, and chronic disease monitoring.
• Develop and update individualized care plans, communicate with physicians and care teams, and document visits in the EMR.
• Provide care in patients’ homes and use a valid driver’s license and reliable transportation to reach local cases.
