The position supports NeighborHealth’s Nutrition Department and its health center wellness and chronic-condition management work. The role contributes to evidence-based medical nutrition therapy and coordinated care through collaboration with participants, caregivers, and the interdisciplinary team.

What you'll do

• Conduct clinical nutrition assessments and develop care plans
• coordinate interventions for nutritional health and chronic-condition management
• complete assessments at enrollment, annually, and when status changes
• oversee nutrition-related competencies for Dietary Assistants and PACE Health Aides
• collaborate with participants, caregivers, and the interdisciplinary team
• refer participants to appropriate services
• oversee PACE Center meals, home-delivered meals, and grocery delivery
• maintain state and national registration or licensure.
