The Case Manager provides mental health rehabilitation and coordinated support to adults and families, helping reduce hospitalization and crisis episodes and strengthen community functioning. Through case management, counseling, crisis intervention, skills training, and coordination with community resources, the role supports clients’ recovery goals and continuity of care.

What you'll do

• Provide case management for designated clients
• conduct individual, group, and family counseling and educational sessions
• provide crisis intervention, skills training, and medication monitoring as part of the Community Support Team
• complete timely documentation and maintain service records
• coordinate community resources and care
• participate in team meetings
• present workshops or training as needed
• maintain confidentiality
• be available for court appearances on behalf of clients.
