Community Integration Specialist

The specialist helps people served pursue their preferred lifestyles and build independence, productivity, and community participation through person-centered planning, daily program activities, community experiences, and individualized support.

What you'll do

• Support person-centered planning
• help individuals identify and participate in inclusive community activities
• teach community and life skills
• provide supervision and assistance with daily living and health needs
• transport individuals to and from home and activities
• administer medication and respond to emergencies as required
• document daily activities, attendance, behavioral data, and regulatory information
• communicate with team members and families
• support staffing and work-related skills.
