The role develops individualized plans, connects clients with resources and job opportunities, tracks progress, and coordinates with employers and service providers to support placement and career readiness.

What you'll do

• Develop and implement individualized vocational rehabilitation plans
• assess barriers related to education, housing, mental health, and employment
• connect clients with community resources, training programs, and employment opportunities
• monitor progress and maintain case notes and service plans
• collaborate with employers, community partners, and service providers
• facilitate job-readiness, life-skills, and career-development workshops and coaching
• meet program documentation, compliance, and reporting requirements.
