Job overview

The Case Manager I/II position is full-time and onsite at locations in Okmulgee and Sapulpa, OK. Annual salary is $40,000.87 for Level I or $43,000.94 for Level II, plus full state employee benefits. The role is with Oklahoma Human Services and serves individuals with intellectual and developmental disabilities in home or community living settings. Under a case management supervisor, the case manager gathers social histories, develops and secures community or agency resources, serves as a Qualified Intellectual Disabilities Professional, and monitors programs monthly. The work helps meet consumers’ needs while maintaining compliance with federal Home and Community-Based Waiver regulations, state requirements, and policy guidelines.

What you'll do

  • Provide services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives
  • obtain in-depth social histories
  • develop and assure community or agency resources to meet consumer needs
  • serve as QIDP
  • monitor the program monthly
  • comply with federal Home and Community-Based Waiver regulations, state requirements, and policy guidelines.
