What you'll do

• Maintain a caseload of high-risk tenants
• provide Housing First case management using evidence-based practices
• conduct assessments and intakes
• develop and update person-centered service plans
• connect residents with benefits and community resources
• coordinate appointments, aftercare, and workforce services
• conduct outreach and apartment visits
• facilitate groups
• maintain required documentation and compliance
• coordinate with property management.
