What you'll do

• Conduct Start of Care admissions and OASIS assessments
• complete follow-up and evaluation visits
• visit patients in the designated territory
• reconcile medications and provide wound care, treatments, and procedures
• monitor patient conditions and report changes
• collaborate with physicians and agency staff
• administer IV medication infusions according to protocols
• document observations and care.
