The Registered Nurse (RN) — Home Health role is onsite in Casper, WY, with compensation of $36–$45 per hour. SNFOut’s home health team helps seniors live well and return to activities they value. The nurse provides skilled care to seniors in their homes, develops individualized care plans, and coordinates with patients, families, and interdisciplinary colleagues. The role supports patient health through assessment, prescribed treatments, education, and accurate documentation.

What you'll do

• Develop and implement individualized care plans
• assess seniors’ physical, emotional, and social needs in their homes
• administer prescribed medications, treatments, and therapies and monitor responses
• educate patients and families on disease management, self-care, and wellness
• coordinate services with healthcare professionals
• maintain detailed patient records and care plans.
