What you'll do
- Provide acute-care hospital case management
- perform comprehensive patient assessments
- coordinate care with multidisciplinary teams
- develop discharge plans and coordinate transitions and post-discharge services
- communicate with patients and families
- monitor progress and discharge barriers
- maintain case management documentation
- and follow hospital policies. The approximate patient ratio is 1:20.
