Registered Nurse (RN) Case Manager

What you'll do

• Review patient history and conduct case screening within 24 hours of admission
• interview patients and significant others about health and psychosocial needs
• review status and treatment plans
• identify barriers and resources
• assess post-acute care environments and teaching needs
• develop individualized care management plans with patient and family input
• identify services, treatments, and funding options
• determine targeted length of stay.

Why this role
