What you'll do

• Coordinate individualized care plans with physicians, families, and caregivers
• monitor services and progress toward goals
• accompany clients to appointments or hospital visits and relay updates
• assist with caregiver hiring, onboarding, and training
• schedule caregivers for a caseload of approximately 30–40 clients
• document assessments, incidents, and progress in EMR/CRM systems
• participate in the shared 24/7 on-call rotation.
