What you'll do
- Assess client needs, strengths, and risk factors
- develop and update transition plans
- connect clients with housing, job training, mental health, substance-use treatment, and benefits resources
- conduct follow-ups and monitor progress
- provide crisis intervention
- advocate with agencies and community organizations
- facilitate life-skills, job-readiness, and personal-development groups
- maintain protected health information and timely electronic medical records
- represent Bridgeways in community activities as requested.
