The Registered Nurse (RN), Nurse-Family Partnership Home Visitor role is an onsite, full-time position in Charlotte, NC, with a stated salary range of $63,000–$65,000 per year. Care Ring’s Nurse-Family Partnership supports first-time mothers and their babies through home visits from early pregnancy until the child’s second birthday. The nurse provides comprehensive nursing services to eligible women and families, following the Nurse-Family Partnership model. Through assessment, education, referrals, and goal-setting support, the role helps families address their needs and work toward their goals.

What you'll do

• Conduct home visits for an assigned caseload of 25 clients
• assess physical, emotional, social, and environmental needs
• build therapeutic relationships using reflection and motivational interviewing
• help families establish goals
• provide education, support, and referrals
• document care and maintain confidentiality
• participate in supervision, case conferences, team meetings, quality improvement, training, and outreach.
