What you'll do

• Manage a client caseload focused on primary care screening and monitoring
• ensure initial and ongoing screenings
• conduct screenings when needed
• obtain and enter available PCP screening records into the EMR
• provide health education
• make and track referrals
• assist with appointment scheduling
• monitor attendance
• coordinate care with providers with client consent
• encourage adherence to PCP recommendations
• document services and collaboration.
