What you'll do

• Perform initial nursing assessments for new admissions
• complete, coordinate, and revise plans of care
• collaborate with appropriate staff to maintain continuity
• communicate patient progress to the clinical manager, physician, and care team
• maintain accurate and timely patient records
• document pain and symptom-management changes and outcomes
• educate families
• make referrals to other hospice disciplines or document why a referral is not made
• and perform assigned on-call responsibilities and services.
