What you'll do

• Assess residents through observation, record review, and interviews
• complete and transmit MDS assessments on time
• prepare or assist with CAT and CAA reports
• collaborate with the interdisciplinary team on individualized care plans
• enter and update care plans
• communicate with residents, families, physicians, and healthcare professionals
• provide floor nursing care when needed
• promote resident rights and report suspected abuse, neglect, or exploitation
• review daily reports for changes in resident condition
• coordinate MDS schedules with facility departments.
