Housing Case Manager

The Housing Case Manager role is onsite in Stanton, CA. Pay is $22–$25 per hour, depending on experience, and the schedule is 8:00 a.m.–4:30 p.m. The role supports Illumination Health + Home’s housing services for people experiencing or at risk of homelessness. The case manager coordinates individualized housing plans, client support, and community resources to help clients build independent living skills and achieve housing stability and permanent housing. The work includes advocacy, crisis intervention, and collaboration with care teams and landlords.

What you'll do

• Manage a caseload of 30–35 clients
• develop housing plans and conduct assessments
• engage clients weekly and in person at least monthly
• document contacts in the EHR
• connect clients with community resources and referrals
• assist with housing applications and transportation to property viewings
• attend care-team meetings
• provide crisis intervention
• coordinate with landlords and property managers
• maintain client confidentiality.
