Care Coordinator – Teen STAR

The role contributes by teaching and practicing skills, connecting people with community supports, and helping them work toward individualized treatment goals.

What you'll do

• Educate individuals served about managing mental illness and/or substance use disorders
• provide transportation when medically necessary and appropriate
• connect individuals with community resources and natural supports
• help develop interpersonal, social, family, community, and independent-living skills
• complete documentation promptly
• follow applicable evidence-based practice models and agency policies
• demonstrate working knowledge of diagnostic criteria
• communicate information accurately
• support compliance with certification standards and the agency strategic plan.
