What you'll do

• Provide patient-centered therapy interventions
• document screenings, psychosocial assessments, progress, and referrals in the electronic medical record
• develop treatment plans collaboratively with patients, families, and care teams
• coordinate with treatment teams and community agencies
• provide individual, group, and family therapy
• monitor progress and support discharge planning and referrals
• participate in staff meetings and case presentations
• provide supervision or consultation to staff and interns as appropriate.
