The nurse manages a primary hospice caseload, assesses and reassesses nursing needs, develops and updates care plans with the Hospice Interdisciplinary Team, and teaches and supports patients, families, caregivers, and staff.

• Provide skilled professional nursing care as prescribed by physicians and in accordance with applicable standards
• conduct initial and comprehensive assessments
• identify patient needs
• collaborate on and implement the interdisciplinary plan of care
• reevaluate and revise care plans
• teach, supervise, and counsel patients, families, and staff
• supervise hospice aides and homemakers.
